Passport replacement – Damaged

  • US passport that are damaged or in poor condition should be replaced even if there is validity remaining in them.
  • Signifacantly damaged passport are automatically invalidated and can no longer be used for international travel.
  • Passport with only normal wear and tear can be just renewed

Important:

  • The ABC Passport Request Form and ABC Authorization Form must be included with every service request.
  • All signatures must be original. Digitally printed signatures are not accepted by the U.S. Department of State.
    1. Application form DS-11 (Note: Do Not Sign until you are in front of the Acceptance Agent.)
    2. Proof of Citizenship — Original or certified copy (no photocopies or notarized copies). Please provide one of the following:
      • Official Birth Certificate (if born in the US), Or
      • Most Recent Passport, Or
      • Naturalization Certificate (for US citizens through naturalization), Or
      • Certificate of Citizenship (if born abroad to US parent(s))
    3. One (1) passport size picture (2 inches by 2 inches, color, plain light or white background, full front view of face. No Glasses.)
    4. The US Passport Agency charges in the amount of $190.00 (Money Order payable to the U.S. Department of State)
    5. A copy of the plane ticket or travel itinerary showing the departure date and destination. Please provide two (2) copies of one of the following:
      • Copy of plane ticket showing foreign departure, Or
      • Copy of travel itinerary (from airline or travel agency with applicant name, travel date, and flight info), Or
      • If travel is for business and neither of the above is available, please send a business travel letter.
    6. Two (2) copies of the Driver’s License or State ID.
      • Applicants age 16 or 17 without a valid ID can have a parent co-sign the application. In this case, a birth certificate proving parental relationship is required.
    7. Capitol Visa Services Authorization Letter — inside the sealed envelope (Letters of Authorization MUST have an original signature! Scanned, Faxed, or Photocopied letters are NOT accepted by the United States Passport Agency.)

APPLICANT WITH EXPIRED PASSPORT 5 YEARS AFTER EXPIRATION DATE:

Same as the above mentioned except the # 2 requirements (Original Birth Certificate) should be replaced by the Expired Passport (which is 15 years from the issuing date).

REQUIREMENTS FOR LOST OR STOLEN PASSPORT:

In addition to the above mentioned for an applicant who never held a US passport before from (1 to 10).

    Minors who have been issued a five-year validity passport must always apply for a first-time passport. Children under the Age of 14 must submit in addition a DS-3053 form along with the following letter of authorization filled out by the parent or legal guardian.
 A Clerk of court/ or a Post Master: CLICK HERE TO FIND THE NEAREST LOCATION (form DSP-11) must be signed in front of a Clerk / or a Post Master and endorsed by them.

You must then request the Clerk or Postmaster that this application is placed in a sealed envelope, it should be only opened by the Passport Authorities.

Don’t open this envelope. Do not leave the envelope with the Clerk or a Postmaster.

Please write your complete name, date of birth, and departure date directly on the sealed envelope. Enclose the sealed envelope in a separate Federal Express envelope. (Please call FedEx at 1-800-463-3339 and specify priority first morning delivery on the air bill)

Please fill out the request form and send a copy to ABC VISA & PASSPORT SVCS along with the sealed envelope. The accuracy and the completion of this form will assist us to secure your passport in the timeliest manner.

    Proof of U.S. Citizenship:
    When applying for a U.S. passport using form DS-11, you must include evidence of U.S. citizenship. All citizenship documents submitted will be returned to you.

    Primary evidence of U.S. citizenship includes an original certified birth certificate issued by a U.S. city, county, or state. Alternatively, an original or certified "long form" birth certificate from the Office of Vital Statistics, showing both parents' full names, with a raised seal is acceptable. Certified birth certificates lacking both parents' full names will not be considered as evidence of U.S. citizenship.

    • You can obtain a certified birth certificate from the Office of Vital Statistics in your state of birth or through an order.

    Hospital-issued birth certificates are NOT valid for applying for a U.S. Passport. Additionally, certified copies of birth certificates must meet specific requirements to be considered acceptable primary evidence of U.S. citizenship.

    • Applicant’s full name
    • Date of birth
    • Place of birth
    • Seal of issuing authority (raised, embossed, impressed, or multicolored)
    • Registrar’s signature
    • Date the certificate was filed with the registrar’s office (must be within one year)

    For those naturalized:

    • Original certificate of citizenship or FS-240, DS-1350 (certificate of birth abroad)
    • Original certificate of Naturalization
