Although you’re not applying online, you’ll complete the questions by typing them onto the application using the U.S. State Department website & ‘Wizard”. After completion, print the summary page and sign it with blue or black ink. The application Wizard will guide you through selecting fees, shipping, and processing options, but no payments will be made, and selected options won’t affect the service provided.
A correctly formatted application should include a 2D barcode on the top left corner and bottom of each page, with “DS-11” printed on the bottom left of the form.
