REQUIREMENTS FOR PASSPORT RENEWAL:

Important:

  • Authorization Form 1 and Authorization Form 2 must be included with every service request.
  • All signatures must be original. Digitally printed signatures are not accepted by the U.S. Department of State.
Form: US DS – 82 Eligibility: You should answer ‘Yes’ to all the following to apply for application Form: US DS-82
You can apply for a Passport Renewal only if:
  • You will be able to submit your most recent US Passport
  • Your most recent passport was issued-dated when you were 16 years of age or above.
  • If your most recent passport is Expired, but issued-dated within the past 5 years, you may still use Form: DS-82.
  • You need additional visa pages due.
  • Your passport was issued a year prior to a legal name change.
  • Your passport is not destructively damaged or unrecognizable.

(U.S. Citizens Note: If you have two (2) U.S. passports, second passports CANNOT be renewed. You must complete the second passport application process.)

  1. Your most recent U.S. Passport:
    1. Submit your most current passport with your DS – 82 application.
    2. Your most current passport cannot be issued more than 15 years ago (In other words, it was issued less than 15 years ago).
    3. The Passport owner must have already reached their 16th birthday before the Passport Issue Date.
  2. A completed and signed application form (DS-82)
  3. One (1) ‘Passport-Style’ Photo, Size: 2 x 2 with White Background (less than 90 days old) (Click here for U.S. State Department Requirements)
  4. The US Passport Agency charges in the amount of $ 212.05 (Check or Money Orderpayable to the US Department of State.  Please do not abbreviate.
  5. Has Your Name Legally Changed? If so, and your passport was issued over a year ago and you need to make an amendment, such as a name change, you’ll need to provide the necessary documentation for a name change through marriage or court order. All documents for a name change or data correction must have an official seal from the issuing authority. Photocopies and notarized documents are not valid for submission to the U.S. Passport Agency.
  6. An authorization letter 1 and authorization letter 2 with original signature from the applicant. (Letters of Authorizations MUST have a hard signature! Scanned, Faxes, or Photocopies are NOT accepted by the United States Passport Agency.)

Passport Card:

When renewing your U.S. passport, mark the box for “Passport Book”; if you’re applying for a regular passport book only.

If you also want a Passport Card, mark the box for “Passport Card” and note there’s an additional $30.00 payable to the US Department of State.  Please do not abbreviate. Passport Cards cannot be expedited currently and takes around 2-3 weeks to be issued. They’ll be processed separately from your regular passport book and mailed to you directly via First Class mail from the U.S. Passport Agency.

The Passport Card is an additional, wallet-sized form of official government ID. It allows for convenient travel by land or sea to Canada, Mexico, Bahamas, and the Caribbean without carrying your Passport Book. The Passport Card remains valid for 10 years.
