FIRST TIME APPLICATION FOR U.S. PASSPORT – Children and Minors (under 16 Years Old):

Important:
  • Authorization Form must be included with every service request.
  • All signatures must be original. Digitally printed signatures are not accepted by the U.S. Department of State.
  1. Applicants Who Have Never Held a U.S. Passport
  2. Application form DS-11 ( Form will be provided upon completing your application online)
  3. Original Birth Certificate (no copies).
  4. One (1) passport size picture (2 inches by 2 inches and White Background)
  5. The US Passport Agency charges in the amount of $ 160.00 (Check or Money Order) payable to the US Department of State.  Please do not abbreviate.
  6. A copy of the airline ticket or travel itinerary showing the departure date and destination
  7. A copy of the driver license or state ID front and back.
  8. Authorization letter inside the sealed envelope (Letters of Authorizations MUST have a hard signature! Scanned, Faxes, or Photocopies are NOT accepted by the United States Passport Agency.) 

THESE INSTRUCTIONS ARE ADDITIONAL REQUIREMENTS FOR CHILDREN UNDER 16 YEARS

Additional Requirements:

1. Both parents or child’s legal guardian must:

  • Present evidence of child’s U.S. citizenship, and
  • Present evidence they are the child’s parents or legal guardian, and
  • Show valid government issued identification, and
  • Sign and take oath before an authorized passport acceptance agent.

2. If the second parent is unavailable, the appearing parent must be able to provide all requirements listed above. In addition, appearing parent must print and complete the following form:

Form: DS-3053: Statement of Consent – Issuance of a Passport to a Minor Under Age 16

This form is required to be submitted for minors under age 16 as evidence of parental permission for the following reasons:

  • You are a non-appearing parent or guardian providing consent for passport issuance for your child, or
  • You are an appearing parent or guardian, and you cannot obtain the written consent of the non-appearing parent or guardian.

3. If no parent is available to sign, the third party in lieu parent must:

Appear with a notarized statement or affidavit from both parents or custodial parent(s) allowing a third party to apply for the passport. If only one parent provides the statement, evidence of sole custody is required. All applications must be signed under oath, as mandated by law.

All minor children under 16 years old, applying for passports are required to appear in-person at a local U.S. Passport acceptance agency.

The U.S. Passport Agency now requires the use of an online application form for all expedited cases, with no handwritten forms accepted. Failure to use the online application will result in processing delays.

Although you’re not applying online, you’ll complete the questions by typing them onto the application using the U.S. State Department website & ‘Wizard”After completion, print the summary page and sign it with blue or black ink. The application Wizard will guide you through selecting fees, shipping, and processing options, but no payments will be made, and selected options won’t affect the service provided.

A correctly formatted application should include a 2D barcode on the top left corner and bottom of each page, with “DS-11” printed on the bottom left of the form.

The DS-11 and Form 3053

  • Print TWO copies of the completed DS-11 and ONE COPY of Form 1053. Do not sign the DS-11 until you are instructed to do so by the acceptance agent / Postmaster. Give one DS-11 and notarized 3053 form to the acceptance agent / Postmaster to be included inside the white sealed envelope. (Do NOT use a shipping envelope!)
  • Finally, The entire white sealed envelope must be mailed to our office for processing.

IMPORTANT:

Please make sure to select the appropriate boxes ON THE AUTHORIZATION LETTERS as seen below:

The Authorization Form MUST BE INSIDE the sealed envelope.

 

9. $35.00 Execution fee paid to the authorized agent to execute the DS-11 Application form.

 

A Clerk of court/ or a Post Master: CLICK HERE TO FIND THE NEAREST LOCATION (form DSP-11) must be signed in front of a Clerk / or a Post Master and endorsed by them.
You must then request the Clerk or Postmaster that this application is placed in a sealed envelope, it should be only opened by the Passport Authorities.

Don’t open this envelope. Do not leave the envelope with the Clerk or a Postmaster.

Please write your complete name, date of birth, and departure date directly on the sealed envelope. Enclose the sealed envelope in a separate Federal Express envelope. (Please call FedEx at 1-800-463-3339 and specify priority first morning delivery on the air bill)

Proof of U.S. Citizenship:
When applying for a U.S. passport using form DS-11, you must include evidence of U.S. citizenship. All citizenship documents submitted will be returned to you.

 

  • You can obtain a certified birth certificate from the Office of Vital Statistics in your state of birth or through an order.

Primary evidence of U.S. citizenship includes an original certified birth certificate issued by a U.S. city, county, or state. Alternatively, an original or certified “long form” birth certificate from the Office of Vital Statistics, showing both parents’ full names, with a raised seal is acceptable. Certified birth certificates lacking both parents’ full names will not be considered as evidence of U.S. citizenship.

 

  • Hospital-issued birth certificates are NOT valid for applying for a U.S. Passport. Additionally, certified copies of birth certificates must meet specific requirements to be considered acceptable primary evidence of U.S. citizenship.
  • Applicant’s full name
  • Date of birth
  • Place of birth
  • Seal of issuing authority (raised, embossed, impressed, or multicolored)
  • Registrar’s signature
  • Date the certificate was filed with the registrar’s office (must be within one year)

For those naturalized:

  • Original certificate of citizenship or FS-240, DS-1350 (certificate of birth abroad)
  • Original certificate of Naturalization

Renewing a Second Passport:
When renewing a second valid passport, include your most recently issued primary (10-year) passport and the expired or expiring second valid passport, if applicable. Whether applying for a new second passport or renewing an expired one, you must also submit your regular 10-year passport.

 

Passport Card:
When renewing your U.S. passport, mark the box for “Passport Book” if you’re applying for a regular passport book only.

If you also want a Passport Card, mark the box for “Passport Card” and note there’s an additional $30.00 Passport Agency fee. Passport Cards cannot be expedited currently and take around 2-3 weeks to be issued. They’ll be processed separately from your regular passport book and mailed to you directly via First Class mail from the U.S. Passport Agency.
